Job Summary
Evaluate and treat older adults in their private residences or independent living facilities by developing, implementing, and modifying individualized physical therapy programs. Conduct home and community mobility assessments, screen patients, and instruct caregivers in skills to promote independence. Manage referrals to other disciplines, order supplies, and maintain equipment while ensuring adherence to clinical documentation and safety standards. Supervise assistants and students as required. Perform routine reexaminations to progress treatment plans and incorporate health and wellness programming. Requires a valid PT license, Master's degree, and reliable transportation. Full-time role with $5,000 bonus, medical/dental/vision benefits, and PTO.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
