Job Summary
Evaluate and treat older adults in home and independent living facility settings through assessment, diagnosis, and individualized rehabilitation programs. Implement therapeutic techniques including manual therapy, gait training, and functional exercises while instructing patients and caregivers on mobility skills. Coordinate referrals to other disciplines, adapt treatment plans based on patient progress, and maintain comprehensive clinical and billing documentation. Supervise therapy assistants and support personnel as needed, ensuring adherence to safety protocols and department schedules.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
- Must be able to lift 50 lbs
