Job Summary
Evaluate, diagnose, and treat older adults in their private residences or independent living facilities through outpatient rehabilitation services. Develop and implement individualized therapy programs covering therapeutic exercise, manual therapy, functional training, and mobility assessments to restore independence. Screen patients, manage case plans, and instruct caregivers on skills essential for safe home and community mobility. Supervise therapy assistants and support personnel while adhering to clinical documentation and safety standards. Order equipment, maintain facilities, and coordinate referrals across disciplines to address comprehensive patient needs.
Required Qualifications
- valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- valid driver's license or reliable, independent transportation to and from the patients' residence
