Job Summary
Provide in-home outpatient physical therapy services to older adults, evaluating and treating patients in the patient’s home or in an independent living facility. Develop and implement individualized PT programs to restore, reinforce, and enhance function; refer to other disciplines as needed; educate patients and caregivers; supervise Physical Therapy Assistants, students, and support personnel per licensure requirements; ensure timely documentation and safe operations; participate in care conferences and team collaboration; adapt plans based on patient needs and promote safe mobility and independence in home and community settings.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice
- Master’s degree in Physical Therapy or Master’s degree in a Physical Therapy related field in addition to a Bachelor’s degree in Physical Therapy; or Bachelor’s degree in Physical Therapy plus 5 years of progressive PT experience
- Qualifying Physical Therapy degree earned outside the U.S. must be evaluated to the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver’s license or reliable transportation to and from patients’ residence
