Job Summary
Provide in-home outpatient physical therapy to an older adult population, developing and delivering individualized PT programs in patients' homes or in independent living facilities. The PT assesses needs, plans and delivers therapy, coordinates referrals to other disciplines, educates patients and caregivers, supervises staff as applicable, and maintains documentation; the role supports a flexible schedule in various home settings and involves mobility, functional training, and wellness-focused therapy.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master’s degree in Physical Therapy; or Master’s in a Physical Therapy related field plus Bachelor’s in Physical Therapy; or Bachelor’s in Physical Therapy plus 5 years of PT experience
- Qualifying Physical Therapy degree earned outside the U.S. must be evaluated to U.S. equivalent
- Must have a valid driver’s license or reliable transportation to/from patients’ residence
