Job Summary
Outpatient in-home physical therapy for older adults, with in-home evaluation and treatment in patients’ residences or independent living facilities. Responsibilities include developing and delivering individualized PT programs, coordinating with the rehabilitation team, supervising assistants and students as needed, ensuring timely documentation, and promoting safety and independence for patients. Requires a valid PT license, Master’s degree (or equivalent combination), and the ability to travel to patients’ homes; visa sponsorship and relocation support are available. Flexible schedule and mixed in-home settings across the service area.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master’s degree in Physical Therapy; or Master’s in a Physical Therapy related field with Bachelor’s in Physical Therapy; or Bachelor’s in Physical Therapy plus 5 years PT experience
- Qualifying PT degree earned outside the U.S. must be evaluated for US equivalence
- Must have valid driver’s license or reliable transportation to/from patients’ residence

- Master's degree in Physical Therapy or related field; Bachelor’s degree in Physical Therapy with 5 years of progressive PT experience as an alternative path; active Physical Therapy license; valid driver's license or reliable transportation
