Job Summary
Evaluate and treat older adults in-home or in independent living facilities, delivering personalized physical therapy programs to restore function and promote independence. Conduct comprehensive screenings, including history reviews and functional assessments, to establish diagnoses and design individualized treatment plans covering therapeutic exercise, manual therapy, and adaptive device training. Direct activities to enhance mobility, instruct patients and caregivers on essential skills, and make referrals to other disciplines for comprehensive care. Supervise assistants and support personnel while ensuring adherence to clinical documentation, safety protocols, and department schedules. Maintain equipment, order supplies, and participate in staffing conferences to optimize patient outcomes.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
