Job Summary
Provide outpatient rehabilitation services in an in-home setting for the older adult population, including private residences and independent living facilities. Evaluate, diagnose, and design individualized physical therapy programs to restore function and promote independence. Implement therapeutic techniques such as gait training, manual therapy, and modalities while instructing patients and caregivers in mobility skills. Screen, examine, and reexamine patients to modify treatment plans, make referrals to other disciplines, and ensure comprehensive care. Supervise Physical Therapy Assistants, students, and support personnel as necessary. Maintain accurate clinical and billing documentation, adhere to department schedules, and ensure a safe environment by managing equipment and identifying hazards.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
