Job Summary
Conduct outpatient physical therapy services in home and independent living facility settings for older adults. Evaluate patients, develop individualized treatment plans, and implement therapeutic techniques including gait training, manual therapy, and functional mobility assessments. Instruct patients and caregivers on skills to promote independence, manage chronic diseases, and reduce fall risks. Supervise therapy assistants and students while ensuring adherence to clinical documentation and safety standards. Collaborate with multidisciplinary teams to coordinate comprehensive care and adapt programs based on patient progress.
Required Qualifications
- valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- valid driver's license or reliable, independent transportation to and from the patients' residence
