Job Summary
Evaluate and treat older adults in private residences or independent living facilities, developing and delivering individualized physical therapy programs to restore function and promote independence. Screen, examine, and assess patients to establish diagnoses, design treatment plans, and implement therapeutic techniques including gait training, manual therapy, and adaptive device application. Instruct patients and caregivers on mobility skills and safety, while making referrals to other disciplines to address comprehensive needs. Supervise therapy assistants and support personnel as applicable, ensure timely clinical documentation, and maintain equipment standards. Provide flexible outpatient coverage with a valid state license and reliable transportation.
Required Qualifications
- valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- valid driver's license or reliable, independent transportation to and from the patients' residence
