Job Summary
Evaluate, diagnose, and treat older adults in their private residences or independent living facilities with a focus on restoring mobility and independence. Develop and implement individualized physical therapy programs including therapeutic exercise, manual therapy, and adaptive device training while instructing patients and caregivers on essential skills. Screen patients for fall risks, manage chronic disease, and coordinate referrals to other disciplines to address comprehensive needs. Supervise therapy assistants and support personnel, maintain accurate clinical and billing documentation, and ensure adherence to safety standards and department schedules.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
