Job Summary
Conduct outpatient rehabilitation services in in-home settings for older adults, including private residences and independent living facilities. Evaluate, diagnose, and develop individualized therapy programs to restore function, while instructing patients and caregivers in essential skills for independence. Implement therapeutic techniques such as gait training, manual therapy, and electrotherapeutic modalities, alongside adaptive device fitting and falls prevention strategies. Supervise therapy assistants and students, manage referrals to other disciplines, and maintain comprehensive clinical and billing documentation. Ensure adherence to safety standards and participate in staffing conferences to coordinate patient care.
Required Qualifications
- valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- valid driver's license or reliable, independent transportation to and from the patients' residence
