Job Summary
Conduct outpatient physical therapy services in patients' homes and independent living facilities, evaluating, diagnosing, and developing individualized rehabilitation programs to restore function and promote independence. Screen and examine patients using appropriate tests to establish baseline status, design treatment plans including therapeutic exercise, manual therapy, and functional training, and instruct patients and caregivers in mobility skills and adaptive device use. Direct activities to maintain health, perform home and community mobility assessments, and make referrals to other disciplines to address comprehensive patient needs. Supervise Physical Therapy Assistants and support personnel, ensure timely clinical documentation, and maintain equipment safety.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
