Job Summary
Evaluate, diagnose, and treat older adults in their private residences or independent living facilities by developing individualized physical therapy programs. Screen patients, apply therapeutic techniques including gait training and manual therapy, instruct caregivers on mobility skills, and adapt plans based on progress. Manage referrals to other disciplines, order equipment, and maintain accurate clinical and billing documentation. Adhere to department schedules, attend care conferences, and ensure a safe environment while promoting patient independence. Requires a valid state license, Master's degree, and reliable transportation.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
