Job Summary
Evaluate and treat older adults in their private residences or independent living facilities, delivering personalized physical therapy programs to restore function and promote independence. Screen, examine, and assess patients to establish diagnoses, identify impairments, and design individualized treatment plans including therapeutic exercise, manual therapy, and adaptive device training. Collaborate with caregivers to educate on chronic disease management and falls prevention, while making referrals to other disciplines to address comprehensive patient needs. Supervise assistants and students, maintain accurate clinical and billing documentation, and adapt plans based on patient progress and safety requirements.
Required Qualifications
- valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- valid driver's license or reliable, independent transportation to and from the patients' residence
