Physical Therapist (PT) - Mobile Outpatient

Conduct outpatient physical therapy services in home and independent living facility settings for older adults, including screening, examination, and evaluation to establish diagnoses and impairment levels. Develop and implement individualized treatment plans covering therapeutic exercise, manual therapy, functional training, and mobility assessments while instructing patients and caregivers in skills to promote independence. Manage referrals to other disciplines, adapt plans based on patient progress, and ensure adherence to safety protocols and documentation requirements. Supervise physical therapy assistants and support personnel as needed.
