Job Summary
Provide outpatient physical therapy services in-home for older adults, including screenings, gait training, and manual therapy. Develop individualized treatment plans, instruct patients and caregivers on mobility skills, and refer to other disciplines as needed. Supervise therapy assistants and students while maintaining accurate clinical and billing documentation. Adapt care based on patient progress and ensure safety by identifying hazards and ordering necessary equipment. Maintain a valid license and driver's license to support these duties across various residential and independent living settings.
Required Qualifications
- valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- valid driver's license or reliable, independent transportation to and from the patients' residence
