Job Summary
Evaluate, diagnose, and treat older adults in their private residences or independent living facilities through a comprehensive plan of care. Implement therapeutic exercises, manual therapy, and functional training to restore mobility and independence while instructing patients and caregivers on safety techniques. Screen for fall risks, manage chronic disease, and coordinate referrals to other disciplines to address comprehensive patient needs. Supervise therapy assistants and support personnel, maintain accurate clinical and billing documentation, and adapt treatment protocols based on individual progress. Ensure a safe environment by enforcing safety standards and managing equipment maintenance.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
