Job Summary
Conduct outpatient physical therapy services in the home for the older adult population, including screening, examination, evaluation, and diagnosis. Develop and implement individualized treatment plans involving therapeutic exercise, functional training, manual therapy, and modalities while instructing patients and caregivers in skills to promote independence. Make referrals to other disciplines, adapt plans based on patient needs, and perform home mobility assessments. Supervise assistants and students as necessary, ensure adherence to documentation requirements, and maintain a safe environment by managing equipment and identifying hazards.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- Valid driver's license or reliable, independent transportation to and from the patients' residence
- Must be able to lift 50 lbs
