Job Summary
Evaluate, diagnose, and treat older adults with physical therapy programs delivered in their private residences or independent living facilities. Conduct home and community mobility assessments, screen patients, and design individualized treatment plans incorporating therapeutic exercise, manual therapy, and adaptive device training. Instruct patients and caregivers on skills to promote independence while making referrals to other disciplines and consulting the rehabilitation team. Adhere to department schedules, complete clinical and billing documentation, and maintain equipment and safety standards. Requires a valid PT license, Master's degree, and reliable transportation.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
- Must be available for weekend shifts
- Must be able to lift 50 lbs
