Job Summary
Conduct outpatient physical therapy in-home for older adults, evaluating patients, developing individualized treatment plans, and implementing therapeutic exercises, manual therapy, and functional training. Direct activities to promote independence, instruct patients and caregivers on mobility skills, and perform home and community mobility assessments. Supervise Physical Therapy Assistants, students, and support personnel while adhering to clinical documentation and safety standards. Consult with the rehabilitation team to select appropriate regimens, make referrals to other disciplines, and adapt plans based on patient progress. Order supplies, maintain equipment, and ensure a safe environment. Requires a valid PT license, Master's degree, and reliable transportation. Full-time role with medical, dental, and vision benefits, plus a $5,000 bonus.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
