Job Summary
Establish therapy goals and formulate treatment plans based on evaluation results for patients to restore function, relieve pain, and prevent disability following disease, injury, or loss of a body part. Provide individual, family, and group therapy sessions for clients engaged in outpatient therapy while supervising Physical Therapy Assistants according to organization policy and state regulations. Participate in interdisciplinary group meetings for hospice patients, case conferences for home health patients, and discharge planning. Conduct initial clinical assessments using standardized tools and protocols to facilitate treatment recommendations, then record all pertinent data in patient medical records. Train new staff as needed. Requires a Bachelor's, Master's, or Doctorate in Physical Therapy with a state license and 1-2 years of hospital, rehab, or ambulatory setting experience.

- Physical Therapist [State License]
- Basic Life Support [BLS Certification]
- Experience working as a physical therapist in a hospital, rehab or Ambulatory setting 1-2 years
