Job Summary
PT - Physical Therapist provides direct rehab care in physical therapy to patients upon physician referral in a Life Care Centers of America SNF setting. Follow established treatment plans, establish and modify functional goals, supervise PTAs and Rehab Aides, chart timely, utilize therapy software, and collaborate with an interdisciplinary team to support patient outcomes. Requires graduation from an accredited PT program and active state licensure; CPR certification; openness to new grads; emphasizes patient care, documentation, compliance with policies, and professional integrity in a post-acute care environment.
Required Qualifications
- Graduate of an accredited program in physical therapy (BSPT, MSPT, or DPT)
- Currently registered/licensed in applicable State with active license
- CPR certification upon hire or during orientation
- One year experience preferred; new grads welcome to apply
- Ability to read, write, speak English
