Job Summary
PT provides direct rehab care in physical therapy to patients under physician referral in accordance with Life Care standards, within Skilled Nursing Facilities and post-acute settings. Responsibilities include following established treatment plans, establishing and modifying measurable goals, supervising PTAs and rehab staff, charting and using therapy software, delivering exceptional patient-centered care, collaborating with an interdisciplinary team, ensuring patient safety and confidentiality, and participating in community health initiatives. Requires knowledge of post-acute care practices, strong communication, and proficiency in English; must maintain licensure and comply with company policies. Required Qualifications
- Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT)
- Current active state license to practice physical therapy
- CPR certification (upon hire or during orientation)
