Physical Therapist (PT)

Conduct physical therapy evaluations and establish individualized treatment plans for residents recovering from orthopedic procedures, neurological conditions, illness, and mobility decline. Deliver therapy focused on strength, balance, gait training, mobility, endurance, and pain management while collaborating with PTA staff, nursing, physicians, and interdisciplinary team members to support care goals. Monitor resident progress and adjust treatment approaches as needed, maintaining accurate and compliant documentation.
