Physical Therapist (PT)

Conduct physical therapy evaluations and develop individualized treatment plans for residents in a skilled nursing and post-acute care setting. Deliver treatment focused on mobility, gait training, transfers, strengthening, balance, endurance, and pain management while collaborating with PTA staff, nursing professionals, physicians, and interdisciplinary team members. Monitor resident progress to adjust treatment plans as appropriate and educate families on exercises and safety recommendations. Maintain accurate clinical documentation. Full-time role with $45–$50/hour salary, located in Waterford, CT.
