Job Summary
Seeking a licensed Physical Therapist (PT) to provide skilled therapy services in a Long-Term Care setting focused on restoring mobility, strength, and function. Responsibilities include reviewing physician referrals and medical records, evaluating strength and mobility, developing treatment plans, administering therapeutic exercises, educating patients and families, completing timely Medicare documentation, collaborating with care teams, and supervising PTAs and aides as required.

- Licensed Physical Therapist (PT)
- Current state PT license

- Long-Term Care (LTC) experience preferred
- Strong knowledge of Medicare guidelines
