Job Summary
Physical Therapist (PT) for Home Health & Hospice provides skilled PT services in patients' homes and hospice settings. Evaluate, plan, and implement physical therapy; develop individualized treatment plans; establish goals to improve mobility, strength, balance, endurance, functional independence, and safety; deliver therapeutic exercises, gait and transfer training, balance activities, and neuromuscular re-education; assess and recommend DME and adaptive equipment; educate patients, caregivers, and families on disease processes, fall prevention, home exercise programs, and safety; evaluate home environments and modify for safety; monitor progress and adjust plans; document in electronic medical records; participate in interdisciplinary care conferences; coordinate with physicians, nurses, therapists, and other team members; supervise PTAs as permitted by state regulations; educate on safe patient handling and transfers; collaborate with hospice team to support end-of-life care; travel to patient homes within service area; maintain compliance with Medicare/Medicaid/state standards and HIPAA.
Required Qualifications
- Graduate of an accredited Physical Therapy program
- Current, unrestricted Physical Therapist (PT) license in the state of employment
- Current CPR/BLS certification from the American Heart Association or equivalent
- Valid driver's license
- Current automobile insurance as required by state law
- Reliable transportation for patient visits
- Minimum one (1) year of clinical experience as a Physical Therapist preferred
