Physical Therapist - PT

The Physical Therapist (PT) will evaluate, plan, and implement skilled physical therapy services for patients in home health and hospice settings. Responsibilities include performing comprehensive evaluations, developing individualized treatment plans, establishing measurable mobility and safety goals, and delivering therapeutic exercises, gait and transfer training, balance activities, and neuromuscular re-education. The PT will assess and recommend DME and assistive devices, educate patients and caregivers, evaluate home environments for safety, monitor progress, and document outcomes in the EMR. Collaboration with physicians, nurses, social workers, therapists, and the interdisciplinary team is essential, as is supervising PTAs within state regulations. Additional duties include contributing to hospice care plans to maximize comfort and independence, educating caregivers on safe handling and fall prevention, coordinating care, and participating in care conferences. The role requires travel to patient homes within assigned service areas, adherence to HIPAA and regulatory standards (Medicare/Medicaid), and occasional after-hours coverage as needed.
