Job Summary
Physical Therapist (PT) for Home Health and Hospice: Evaluates patients in home settings, develops and implements individualized treatment plans to improve mobility, strength, balance, endurance, and safety; provides skilled PT, gait and transfer training, neuromuscular re-education, and education for patients and caregivers; assesses environments and recommends equipment/assistive devices; collaborates with physicians, nurses, social workers, and other team members; supervises PTAs as allowed; participates in hospice care focusing on comfort and quality of life; maintains clinical documentation and adheres to Medicare/Medicaid/state standards; must travel to patient homes and possess a valid license, CPR/BLS, and transportation.
Required Qualifications
- Graduate of an accredited Physical Therapy program
- Current, unrestricted Physical Therapist (PT) license in the state of employment
- Current CPR/BLS certification from the American Heart Association or equivalent
- Valid driver's license
- Current automobile insurance as required by state law
- Reliable transportation for patient visits
- Minimum one (1) year of clinical experience as a Physical Therapist preferred
