Physical Therapist - PT

The Physical Therapist (PT) provides in-home physical therapy services for patients receiving home health and hospice care. Responsibilities include evaluating patients, developing individualized treatment plans, establishing measurable goals to improve mobility and safety, performing skilled therapeutic exercises, gait and transfer training, balance activities, neuromuscular re-education, and recommending DME or adaptive equipment. The PT educates patients, caregivers, and family members on disease processes, home exercise programs, fall prevention, and safety techniques; evaluates home environments for safety modifications; monitors progress and adjusts plans; documents in the EMR; participates in interdisciplinary care conferences; coordinates care with physicians, nurses, therapists and other professionals; and may supervise PTAs as allowed by state regulations. In hospice, the PT focuses on maximizing comfort and maintaining functional abilities, assists with pain management strategies, and supports end-of-life care alongside the hospice team. Clinical responsibilities include assessments, care planning, documentation, HIPAA compliance, infection control, and travel to patient homes. Preferred qualifications include experience in Home Health/Hospice, OASIS documentation, Medicare participation knowledge, and bilingual skills.
