Job Summary
Evaluate patient conditions, develop individualized treatment plans, and guide patients through progressive rehabilitation to restore mobility and function. Document all patient care in accordance with regulatory, licensing, and accrediting requirements while maintaining a clean and safe work environment. Collaborate closely with supervisors, interdisciplinary team members, and families to ensure continuity of care and effective follow-through on therapeutic goals. Prioritize patient-centered care for individuals with complex medical conditions requiring extended recovery in a Joint Commission Gold Seal accredited setting.
Required Qualifications
- Degree from an accredited Physical Therapy program
- Current and unrestricted Physical Therapy license in the state where services are rendered
- CPR/Basic Life Support (BLS)
- Candidates must have strong organizational and communication skills for success in this role
Desired Qualifications
- Previous experience in rehab setting preferred
