Job Summary
Provide direct rehabilitation services as a Physical Therapist in Life Care Centers of America SNFs, delivering patient-focused care to diverse post-acute populations. Responsibilities include following established PT treatment plans, establishing and adjusting functional goals, supervising PTAs and rehab staff, accurate charting, and effective use of therapy software. Requires licensure in state, MSPT/DPT preferred (MSPT/DPT required if applicable), CPR certification, and strong teamwork in an interdisciplinary setting. Emphasizes quality of life, safety, confidentiality, compliance with policies, and ongoing professional development in a post-acute care environment.
Required Qualifications
- Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT)
- Currently registered/licensed as a Physical Therapist in the applicable state with active license
- CPR certification upon hire (or obtained during orientation) and must remain current during employment
- Proficiency in Microsoft Word, Excel, and email
