Job Summary
PT - Physical Therapist provides direct rehab care in physical therapy to patients upon physician referral in accordance with Life Care standards. Follow physical therapy treatment plans, establish and modify goals, oversee and evaluate care provided by PTAs and students, chart appropriately, utilize therapy software, and exhibit excellent customer service. Maintains interdisciplinary team communication, participates in community health matters, and adheres to company policies and code of conduct. Must have an active PT license, MSPT or DPT preferred (or BSPT), CPR certification, and be capable of working in a post-acute care facility with a focus on patient rights, safety, and confidentiality.
Required Qualifications
- Graduate of an accredited program in physical therapy (BSPT, MSPT, or DPT)
- Currently registered/licensed in applicable State
- Active license in good standing throughout employment
- CPR certification upon hire or during orientation
- One (1) year experience preferred; new grads welcome to apply
- Proficient in Microsoft Word, Excel, and email
- Knowledgeable of practices and procedures governing post-acute care facility
- Demonstrates ability to implement and interpret department programs and policies
- Ability to read, write, speak, and understand English
