Job Summary
Provide direct rehab care as a Physical Therapist to patients upon physician referral, following Life Care standards. Establish, assess, and modify functional goals; lead care planning and oversee PTAs, Rehab Aides, and students; document via therapy software and maintain timely charting. Demonstrate strong patient-centered care, excellent customer service, and effective interdisciplinary collaboration. Maintain professional conduct, safety, and compliance with policies; support community health initiatives and ongoing education opportunities. Requires an accredited PT degree (BSPT/MSPT/DPT) and active state licensure; CPR current; able to work in a nursing-home post-acute care setting with emphasis on short-term rehab, long-term care, and post-operative recovery.
Required Qualifications
- Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT)
- Current state licensure with active license in good standing
- CPR certification (on hire or during orientation) and current during employment
- 1 year of experience preferred; new grads welcome to apply
