Physical Therapist (PT)

Job Summary
Follow physical therapy treatment plans for patients; establish, assess, and modify goals; oversee and evaluate care provided by PTAs, Rehab Aides, and students; chart appropriately and timely; use therapy software; provide excellent customer service; assist in patient evacuation; maintain regular attendance; work within an interdisciplinary team; comply with policies and maintain confidentiality. Essential functions include evaluating care, documenting patient progress, and ensuring safe, effective delivery of rehab services in a post-acute care setting. An Equal Opportunity Employer.
Required Qualifications
- Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT)
- Currently licensed/registered in applicable State with active license in good standing
- CPR certification upon hire or during orientation (must remain current)
- 1 year experience preferred; new grads welcome to apply
- Proficient in Microsoft Word, Excel, and email
- Ability to communicate with patients, families, and other departments; maintain confidentiality and follow policies
- Demonstrates integrity and professionalism; adheres to Code of Conduct and compliance training
