Job Summary
Provide direct rehab care as a Physical Therapist (PT) in a post-acute care setting. Follow physician-referred treatment plans, establish, assess, and modify functional goals, oversee care delivered by PTAs and Rehab Aides, and accurately chart patient progress. Utilize therapy software, communicate effectively with patients, families, and interdisciplinary teams, uphold patient rights, safety, and confidentiality, and adhere to Life Care’s policies and Code of Conduct. Maintain regular attendance, demonstrate professional integrity, and participate in mandatory compliance training. Licensed PT must maintain active licensure and CPR current throughout employment. New graduates are welcome to apply; one year of experience preferred. Essential functions include patient care delivery, goal setting, supervision of support staff, and reliable documentation.
Required Qualifications
- Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT)
- Current state license/registration with active standing
- CPR certification (upon hire or during orientation) and maintenance of current CPR
- One year experience preferred; new grads welcome to apply
- Proficiency with Microsoft Word, Excel, and email
- Ability to read, write, speak, and understand English
- Demonstrated knowledge of post-acute care facility practices and applicable laws/regulations
- Ability to establish, assess, and modify goals; follow treatment plans; oversee PTAs and Rehab Aides; chart timely
- Ability to maintain confidentiality and adhere to Life Care policies and Code of Conduct
