Physical Therapist (PT)

Provide direct rehab care in physical therapy to patients upon physician referral in accordance with all applicable laws and Life Care standards. Follow established treatment plans, establish and modify realistic goals, supervise PTAs and rehab aides, chart appropriately and timely, utilize therapy software, and maintain excellent customer service. Collaborate on an interdisciplinary team with nursing, therapy, and medical staff; ensure patient rights, safety, and confidentiality; participate in community health activities; demonstrate professional conduct and compliance with Life Care’s Code of Conduct; support ongoing professional development and eligibility for leadership opportunities. The role involves assessing patient progress, coordinating care, and contributing to post-acute and sub-acute rehabilitation across orthopedic, neurological, and cardiopulmonary populations.
