Physical Therapist (PT)

Job Summary
Provide direct rehabilitation therapy to patients per physician referrals in a post-acute care setting. Follow established PT treatment plans, establish and adjust functional goals, supervise PTAs and Rehab Aides, and document care using therapy software. Maintain patient rights, safety, and confidentiality; collaborate with an interdisciplinary team; adhere to Life Care policies and compliance training; and demonstrate professional behavior, reliability, and strong customer service while performing activities like lifting, standing, reaching, and mobility tasks.
Required Qualifications
- Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT)
- Currently registered/licensed in applicable State
- CPR certification
- One (1) year experience preferred; new grads welcome to apply
- Proficient in Microsoft Word, Excel, and email
- Demonstrates good body mechanics
- Follow treatment plans and establish goals
- Oversee care provided by PTAs, Rehab Aides, and students
- Demonstrate confidentiality and professionalism
- Maintain compliance with Life Care policies
