Physical Therapist (PT)

Job Summary
Provide direct rehab care as a Physical Therapist to patients referred by physicians. Follow established PT treatment plans; establish, assess, and modify realistic, measurable goals; oversee care provided by PTAs and students; document appropriately using therapy software; maintain excellent patient service and teamwork; participate in community health matters as appropriate; maintain confidentiality and adhere to applicable laws, regulations, and Life Care standards. Requires licensure in the state, CPR certification, and a degree from an accredited physical therapy program (BSPT/ MSPT/ DPT); new grads welcome and one year of experience preferred. The role emphasizes patient assessment, goal setting, documentation, and collaboration within an interdisciplinary team in a post-acute care facility.
Required Qualifications
- Graduate of an accredited program in physical therapy (BSPT, MSPT, or DPT)
- Currently registered/licensed in applicable State
- CPR certification
- 1 year experience preferred; new grads welcome to apply
