Physical Therapist (PT)

Job Summary
Provide direct rehab care as a licensed Physical Therapist in a post-acute care setting, following physician referrals and established treatment plans. Establish, assess, and modify realistic, measurable goals; oversee care given by PTAs, Rehab Aides, and students; chart appropriately and timely; utilize therapy software; deliver excellent customer service and maintain patient rights, safety, and confidentiality. Participate in community health matters as appropriate; liaise with patients, families, and other departments to plan for patient needs; maintain professionalism and compliance with Life Care’s Code of Conduct; function effectively on an interdisciplinary team and demonstrate good body mechanics and dependable attendance.
Required Qualifications
- Graduate of an accredited program in physical therapy (BSPT, MSPT, or DPT)
- Currently registered/licensed in applicable State with active license
- CPR certification current
- One year experience preferred but new grads welcome
- Proficient in Microsoft Word, Excel, and email
- Ability to read, write, speak English
- Ability to follow company policies and code of conduct
