Physical Therapist (PT)

Conduct direct physical therapy care for patients upon physician referral by establishing, assessing, and modifying realistic, measurable, and functional goals. Oversee and evaluate care provided by PTAs, rehab aides, and students while charting appropriately and utilizing therapy software accurately. Liaise with patients, families, and interdisciplinary teams to plan needs, implement department policies, and demonstrate good body mechanics during patient evacuation and daily activities. Maintain confidentiality, adhere to compliance training, and exhibit excellent customer service within Life Care Centers of America skilled nursing facilities serving long-term and sub-acute rehabilitation populations.
