Job Summary
Provide direct rehab care as a Physical Therapist in Life Care Centers of America SNFs, delivering post-acute rehabilitation and ongoing skilled support. Follow physician-referred treatment plans, establish and modify realistic, measurable goals, and oversee care provided by PTAs and rehab staff. Document appropriately, utilize therapy software, and maintain patient-centered communication with patients and families. Collaborate with an interdisciplinary team to optimize functional progress and quality of life, while upholding confidentiality, company policies, and professional standards. Required Qualifications
- Graduate of an accredited physical therapy program (BSPT/MSPT/DPT)
- Current State license in good standing
- CPR certification (upon hire or during orientation)
- One year post-acute care experience preferred
- Proficient in Microsoft Word, Excel, and email
- Ability to make independent decisions and communicate effectively within an interdisciplinary team
