Physical Therapist (PT)

Physical Therapist provides direct rehab care in physical therapy to patients upon physician referral in accordance with applicable laws and Life Care standards. Follow established treatment plans; establish, assess, and modify realistic, measurable goals; oversee and evaluate care provided by PTAs, Rehab Aides, and PT/PTA students; chart appropriately and timely; utilize therapy software; deliver excellent customer service; collaborate within an interdisciplinary team; maintain patient rights, safety, confidentiality, and professional conduct; demonstrate dependable attendance and strong clinical judgment; contribute to a compassionate, patient-centered environment across long-term and sub-acute care populations.
