Job Summary
Physical Therapist provides direct rehab care in physical therapy to patients upon physician referral in Life Care’s post-acute care settings. Follow established PT treatment plans; establish, assess, and modify functional goals; oversee and evaluate care by PTAs and Rehab Aides; chart timely and accurately; use therapy software; collaborate with an interdisciplinary team; maintain patient rights, confidentiality, and compliance with policies; demonstrate strong body mechanics, judgment, and professional conduct; participate in community health projects as appropriate; support patients and families through coordinated care in SNF/sub-acute rehabilitation settings; opportunities for career advancement within the organization.
Required Qualifications
- Graduate of an accredited physical therapy program (BSPT/MSPT/DPT)
- Currently licensed in applicable State; active license in good standing
- CPR certification current (upon hire or during orientation)
- One year experience preferred; new grads welcome to apply
