Job Summary
Provide direct physical therapy rehab care to patients upon physician referral in a skilled nursing facility setting. Follow established treatment plans, establish and modify functional goals, and oversee care delivered by PTAs and rehab aides. Chart promptly and accurately; utilize therapy software; collaborate with an interdisciplinary team, including nursing and medical staff, to plan patient care. Maintain patient rights, safety, confidentiality, and compliance with company policies; demonstrate professionalism and strong communication with patients, families, and colleagues. A PT with CPT knowledge will assess and contribute to functional progress for orthopedic, neurological, cardiopulmonary, and post-acute populations within Life Care Centers of America facilities. The role requires regular attendance, good body mechanics, and active participation in community health matters, while supporting career advancement and leadership opportunities.
Required Qualifications
- Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT)
- Currently licensed/registered in the applicable state and maintain active licensure
- New grads welcome; one year experience preferred
- CPR certification at hire or during orientation and must remain current
