Job Summary
Evaluate patients based on physician referrals, develop individualized treatment plans with measurable outcomes, and provide treatment in accordance with established plans of care. Monitor patient progress, modify treatment as clinically appropriate, and maintain accurate clinical documentation and charges per billing guidelines. Educate patients and staff on therapeutic techniques, participate in discharge planning, and communicate effectively with the interdisciplinary team regarding progress and concerns. Screen patients for therapy needs, coordinate with nursing and physicians, and supervise PTAs, therapy students, and technicians. Maintain a clean therapy environment, assist with durable medical equipment, and float between nearby facilities as needed.
Required Qualifications
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist license or eligibility for licensure in the state where services are provided
- Strong clinical assessment and treatment skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of an interdisciplinary team
- Strong organizational and documentation skills
- Knowledge of Medicare and other applicable reimbursement requirements
- Commitment to patient-centered care and positive clinical outcomes
- Ability to work effectively with patients, families, and team members
- Ability to stand, walk, bend, stoop, push, pull, and sit for extended periods
- Ability to safely assist, transfer, and support patients
- Ability to lift and carry up to 50 pounds
- Ability to communicate effectively with patients, families, and healthcare professionals
- Ability to maintain appropriate attention to patient safety throughout treatment
- Ability to manage the physical and emotional demands of the position
- Occasional travel between facilities based on patient and operational needs
