Job Summary
Perform initial and ongoing physical therapy evaluations according to APTA and state standards. Develop and implement Plans of Treatment with short and long-term goals, adjusting schedules based on patient progress. Document patient progress and report updates to patients, caregivers, facility staff, and healthcare professionals. Evaluate mobility, strength, and functional levels to set realistic improvement goals. Coordinate with specialists as needed and create discharge plans for in-home care services. Maintain professional knowledge through workshops and publications while assisting staff to ensure a clean, orderly environment. Must pass level 2 fingerprinting with an Eligible status.

- Experience working with a wide range of patient injuries and age groups
- Must be able to pass level 2 fingerprinting with an Eligible status
