Job Summary
Conduct comprehensive initial and ongoing assessments, deliver therapeutic interventions, and communicate with the interdisciplinary team to coordinate patient care and discharge planning. Participate in the Quality Assessment and Performance Improvement program and report patient conditions to the Rehab Director and appropriate staff. Manage a heavy workload by multi-tasking under pressure while maintaining one-on-one patient care in home settings. Requires Colorado PT licensure, BLS CPR, and one year of experience; familiarity with Oasis documentation is preferred.
Required Qualifications
- Current Physical Therapist (PT) licensure in the state of Colorado
- Current Basic Life Support (BLS) CPR
- One year of PT experience
- Pass a background check and drug test
