Job Summary
Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury. Conduct assessments, document progress, and modify care plans while assisting patients with exercises and hands-on therapy to manage pain and increase mobility. Educate patients on self-care and coach families on safely assisting with therapy goals. Work as part of a home health team including RNs, LPNs, CNAs, and OTs to support the patient's plan of care. Maintain CPR certification and knowledge of state and federal home health regulations. Requires an active PT license in CA, one year of experience, and strong clinical judgment.
Required Qualifications
- Graduate of an accredited Physical Therapist Program
- Active PT license in CA
- Minimum of 1 years of physical therapy experience
- CPR certification
- Knowledge of state and federal home health regulations
- Good clinical judgement
- Strong interpersonal skills
